Plants usually stout, to 65 cm, with 4-5 oblong-lanceolate basal leaves. Spike to 30 cm, sometimes very lax, with up to 8 flowers, lowest to 8 cm distant. Sepals ovate to ovate-lanceolate, to 15 x 7 mm, rose-violet tinged, with green mid-vein; dorsal erecto-patent, laterals spreading or ± reflexed.  Petals small, triangular or triangular-ovate, 2.5-3.5 mm, obtuse, rose-purple velvety. Labellum rather large, 10-13 mm, 3-lobed; lateral lobes small, with 4 mm horns slightly bent outwards; middle lobe ovate with recurved margins, shortly brownish-velvety; speculum usually covering half of middle lobe, forming variously lobed yellowish ribbons, encircling 1 ovate velvety zones; appendix stretched fowards, yellowish-green, broadly ovate, often tridentate. Fl. 4-6. Macchie, phrygana, Quercus scrub, Pinus forest, Populus plantations, often in moist places, on calcareous and schistose soil, s.l. -1700 m.
